Axis III – Describes physical disorders and general medical condition

(brain trauma or chemical imbalance that may be causing syndrome)

Axis IV – Measure of environmental stressors and psychosocial problems

(death of spouse, recently fired, lack of support)

Axis V – Global Assessment of Functioning (GAF) – Measurement of an individual’s

quality of life in (3) major areas. Social relationships, occupational
